Neonatal and Paediatric Phlebotomy

Neonatal and Paediatric Phlebotomy

Neonatal and paediatric Phlebotomy refers to the process of obtaining blood samples from newborns (neonates) and children (paediatrics) through a vein.

This procedure requires specialised skills due to the small size of veins, delicate skin, and the need to minimise pain and distress.

Training focuses on minimising pain and discomfort through skilled technique and pain-management strategies, while ensuring safety, accuracy, and infection control.

It is essential for healthcare professionals to perform this with confidence and precision.

Note: This course assumes prior training in adult venepuncture. It is not suitable for complete beginners.

Key Aspects of Neonatal and Paediatric Phlebotomy

  • Purpose
The main goal is to obtain an adequate blood sample for laboratory analysis (haematological, biochemical, bacteriological) with minimal pain and stress to the child. It may also be required for intravenous access (cannulation) for IV therapy.
  • Patient Considerations
Due to smaller body weights and blood volumes, it is vital to minimise blood loss to prevent iatrogenic anaemia and reduce the risk of trauma.
  • Pain Management
The procedure can be stressful and painful, so pain-relief strategies are essential. These may include topical anaesthetics (“magic cream”), oral sucrose for infants, pacifiers, and comfort positioning (e.g., parents holding the child).
  • Technique
Techniques differ significantly from adult venepuncture. Children’s veins are more superficial, requiring a shallower angle of entry. Smaller equipment—such as 23- or 25-gauge butterfly needles and low-vacuum collection tubes—is often used to prevent vein collapse.
  • Site Selection
Common sites include veins in the hand, forearm, or antecubital fossa. In neonates and infants, heel pricks are often used for small-volume capillary samples (e.g., newborn screening), while venepuncture is preferred for larger volumes or specific tests like blood cultures.
  • Psychological Care
Healthcare professionals use distraction techniques, a calm approach, and clear communication (simple language for older children) to reduce anxiety. Involving parents or a play specialist is highly recommended.
  • Professional Skill
The procedure should only be performed by trained and competent practitioners to avoid multiple attempts and prevent complications such as bruising, haematoma, or nerve injury.

In essence, neonatal and paediatric venepuncture combines clinical expertise with compassionate, age-appropriate care to safely obtain necessary samples or establish IV access.

Learning outcomes

By the end of this course, learners will be able to:

  • Explain the anatomical and physiological differences in paediatric patients that affect venepuncture.
  • Identify appropriate veins and select suitable equipment for different age groups.
  • Apply child-centred communication strategies to reduce anxiety and gain cooperation.
  • Perform paediatric venepuncture using safe, aseptic technique.
  • Recognise and respond to complications such as vasovagal episodes, haemolysis, or failed attempts.
  • Document procedures accurately and in line with local protocols and safeguarding requirements.

Who is this course for?

This neonatal and paediatric venepuncture course is designed for registered healthcare professionals and experienced phlebotomists who need to perform blood draws on infants and children as part of their clinical duties.

It is not intended for complete beginners with no prior venepuncture experience.

The course is suitable for a range of professionals, including:

  • Qualified neonatal and paediatric nurses.
  • Doctors.
  • Advanced Neonatal Nurse Practitioners (ANNPs).
  • Experienced phlebotomists.
  • Nursing associates and healthcare support workers (HCAs) with appropriate experience and local approval.
  • Midwives.
  • Medical technicians.

Prerequisites

Most providers have specific entry requirements to ensure participants have a foundational understanding of the procedure and the clinical environment:

  • Prior venepuncture experience: The most common prerequisite is existing experience (typically a minimum of three to twelve months) in adult phlebotomy. Providers often require proof of this experience, such as a professional reference or a certificate from an introductory course.
  • Healthcare background: Candidates are usually expected to be working within a healthcare setting, such as a hospital, clinic, GP surgery, or community care.
  • Mandatory training: Applicants often need to have completed core mandatory training, such as infection control, safeguarding children, and basic life support.

In summary, the course is intended for those who already have a solid foundation in phlebotomy and need to develop the specialist skills and knowledge required for the unique challenges of working with young patients.

Why this training matters

Venepuncture in neonates and children requires more than technical skill—it demands emotional intelligence, regulatory awareness, and trauma-informed care. This course equips team with the skill to:

  • Minimise distress and build trust with young patients and families.
  • Reduce the risk of repeat procedures, safeguarding concerns, and clinical errors.
  • Improve outcomes, confidence, and CQC compliance.
  • Support induction, upskilling, and role extension across paediatric services.

Note: This paediatric phlebotomy course is typically delivered as in-house group training across the UK. We only offer adult phlebotomy training to both individuals and groups. You can talk to our course advisors before booking.
